Penile implant surgery is one of the few cosmetic-adjacent procedures where the relationship between surgeon/hospital case volume and complication rates has been directly studied at scale. A nationwide analysis of nearly 18,000 men undergoing first-time penile prosthesis implantation between 2016 and 2022, published in the journal Urology, found that postoperative infection rates were lower at high-volume hospitals (2.4%) than at low-volume hospitals (3.7%). The study defined high-volume surgeons as those performing more than 33 procedures annually and high-volume hospitals as those performing more than 60 procedures annually, concluding that "these results highlight the importance of surgical experience and hospital resources in achieving better outcomes." This is a meaningfully different risk profile depending on where and with whom you have the surgery, which makes case-volume a fair and specific question to ask directly.

What kind of specialist to look for

Penile implant placement is a urologic surgical procedure, and the Sexual Medicine Society of North America (SMSNA) — the leading professional body for sexual medicine specialists — frames patient readiness around understanding "the risk-benefit ratio" together with a surgeon the patient knows and trusts, rather than prescribing a single certification checklist. In practice, that means looking specifically for a urologist with dedicated training and a meaningful ongoing caseload in prosthetic or reconstructive urology (sometimes described as andrology or sexual medicine), rather than a general urologist for whom implants are an occasional add-on procedure.

Questions worth asking at consultation

• How many penile implant procedures do you personally perform per year? Given the documented volume-infection relationship, this is one of the more evidence-backed questions you can ask in any cosmetic or reconstructive-surgery consultation.
• What is your personal infection rate and revision/reoperation rate?
• Which implant type (malleable vs. inflatable) and brand do you recommend for me, and why?
• What is your antibiotic and infection-prevention protocol during surgery?
• What happens if the implant becomes infected or needs revision later?

This applies whether you're consulting a specialist in Delhi or Kolkata — ask about volume and infection rates directly rather than assuming a hospital's general reputation covers it.

Red flags

• A surgeon who cannot or will not state an approximate annual case volume.
• No clear answer on antibiotic protocol or infection-prevention steps.
